Re: Has anybody else found this bug with 9.1.5, forwarding tasks assigned to single group after another persons takes action on Request ticket. Forwarding to stakeholders and groups referenced in fields still works.

That's correct, the forward_action determines whether or not there are multiple recipients or single recipient.

Please provide your system settings as workflow may behave differently depending on what is/isn't configured. A screenshot of the workflow admin settings will do. Also, are the requests logged from portal or within the main app?
